Statewide System and Organizational Strategy for Evidence-based Practice Implementation and Sustainment in Substance Use Disorder Treatment

This proposed project will test an implementation strategy that seeks to align system, organization, and clinic leadership and strategies to implement and sustain an evidence-based practice (EBP) for substance use disorders. The Leadership and Organizational Change for Implementation - System Level (LOCI-SL) seeks to improve implementation leadership, climate, provider attitudes and behaviors, and EBP fidelity for enhanced client engagement in services and outcomes. LOCI-SL will be tested in integrated mental health and substance use disorder treatment clinics across the state of Oregon.

Aligning facility leadership and climate to advance mental health services integration in Malawi

Task-shared mental health interventions are effective in low-and middle-income countries (LMICs), yet they remain underutilized, and the mental health treatment gap remains substantial. Innovative implementation strategies are needed to successfully integrate evidence-based mental health treatments into medical care in LMICs. This cluster-randomized trial will evaluate the impact of an adapted leadership alignment strategy on the integration of an evidence-based mental health treatment package into multiple medical care settings.

Leadership and Organizational Change Learning Network Hub Site

The purpose of the QUERI Learning Network Hub is to build capacity to deploy and spread an implementation strategy designed to improve organizational leadership to increase the use of evidence-based practices in VA mental health. It will be the first implementation strategy that explicitly focuses on leadership and organizational change in VA mental health. By building capacity for LOCI within VA, the LOCI Hub supports the VA priorities of modernizing systems and focusing resources more efficiently by training, supporting, and motivating leaders to promote the use of evidence-based interventions for veterans. Ultimately, the Hub aims to improve Veteran experience, care quality, and mental health outcomes, all key priorities for VA.

Testing the Leadership and Organizational Change for Implementation (LOCI) Intervention in Substance Abuse Treatment Programs

This study tested the impact of the Leadership and Organizational Change for Implementation (LOCI) intervention in facilitating implementation and fidelity of Motivational Interviewing (MI) in nine substance use disorder treatment agencies at multiple sites across California and Arizona.  Initial support for the feasibility, acceptability, and utility of the LOCI intervention was successfully demonstrated in an NIH supported pilot study.
